[chore] Use consistent approach for overrides

This goes through ensuring that when a span property is overiden by via notes , they are being done so in a consistent with what was being overridden.

This means:

• Consistent phrase for linking to recording error document
• Paragraph prefix is consistent
• Ensuring correct spacing
