Updated cause of death concepts

Removed Concept: "Probable cause of death" (CIEL 1599) (row 67 - 67)
Removed 3 mappings:

1. CIEL → CIEL (self-reference)
2. CIEL → SNOMED-CT (184305005)
3. CIEL → AMPATH (5002)

All of these instances of the deleted concept should be fixed to leverage the newly added "Probable cause of death" concept.

@VeronicaMuthee Usually we want these self-reference mappings, i.e., that's how we "know" that the concept is, e.g., CIEL:1599. The other two mappings should be retained too, I think...

Seems like the issue is with importation of this concept ....160218, let me import that "Caused by: org.openmrs.module.openconceptlab.importer.SavingException: Cannot create mapping from concept with URL /orgs/CIEL/sources/CIEL/concepts/160218/, because the concept has not been imported"
