Fixes #207

  • Remove PDF renderer classes
  • Remove PDF formats from Package_PHP_Factory
  • Update tests to sync with PDF removal: there were no PDF-related tests, but two tests listed pdf and bigpdf in "Supported packages".
  • Remove references to PDF from docs and code comments

Out of scope

  • Did not touch pdf mentions in tests/package/php/data/version.xml. It's unclear how this file is used in this project. We can sort this out in a separate PR.
  • Did not attempt to remove code that possibly became unused as a result. This keeps the PR focused and less risky.
