Update facilitator crud styling

What is the goal of this PR and why is this important?

• Adjust facilitator form so it's talking to only facilitator fields (added phone/best_time_to_call, notes, and dob)
• Adjust users_controller create facilitator to pull from more user fields
• Clean up search text
• Make sectors pills on Facilitators index (will make them clickable later)
• Only show actions column to super users on Facilitators index
• Show user avatar if facilitator avatar_image isn't present and there is one on user (should only be a couple users)
• Change users form to only show comments (bc moving notes from user to facilitator)

How did you approach the change?
