Refactor documentation to reflect hexagonal (ports and adapters) architecture replacing the previous domain-driven design structure. Updated component diagrams to clearly separate Application, Domain, and Infrastructure layers. Updated ADK framework version to 0.3.0 and revised workflow descriptions to match the new pipeline controller implementation. Changes applied to both English and Chinese README files.
