SqueakSSL with SNI (Spur maintenance release)

Nearly the same as v0.2.0@master except for:

• Recompiled all binaries with an updated generated SqueakSSL.c that honors Spur's handling of primitive error codes
• Removed the linux64 part. It is untested
• Linked the Linux binary statically against LibreSSL 2.2.1
