Add initial Claude usage tracking foundation

Overview

Initial groundwork for Claude integration (Phase 1 & 2 skeleton):

• Adds ClaudeLogManager for sandbox-safe log parsing
• Introduces ClaudeLogEntry & FiveHourWindow models
• Extends ServiceProvider with .claude
• Implements ClaudeProvider conforming to existing ProviderProtocol
• Updates ProviderFactory to return ClaudeProvider

These changes compile but UI hooks & tokenizer files will follow in subsequent commits/phases.

Implements parts of #ClaudeIntegration spec.
