refactor: auth0 on top of foundation simulator

Motivation

Moving to build on top of the foundation simulator. This will ease use and setup, and help share code across simulators.

Closes #304.

• refactor server on top of foundation simulator
• switch to starfx based store and set a default user
• tests passing
• deal with cert
• fix typescript build
• test against auth0 example app
• update readme
• squash into nice commits
