⚡ Bolt: Add composite index for comments and name index for machines

💡 What:

• Replaced idx_issue_comments_issue_id with idx_issue_comments_issue_id_created_at (composite index) in issue_comments.
• Added idx_machines_name to machines table.

🎯 Why:

• issue_comments are always fetched by issueId and sorted by createdAt. The composite index avoids a sort operation.
• machines are frequently listed and sorted by name (e.g. in filters). This index avoids O(N log N) sorting at runtime.

📊 Impact:

• Faster loading of Issue Detail pages (comments section).
• Faster loading of Issue List page (machines filter population).

🔬 Measurement:

• EXPLAIN ANALYZE would show Index Scan instead of Sort for these queries.
