fix: Re-initialize WXT modules correctly during development

This close #857, #863, #939.

There were lots of callbacks and dependencies between variables in the createServer function... I was able to clean it up apart of this PR! Very happy about that, wasn't sure how I would work around that...

I don't have E2E tests for dev mode, so we'll need to check over this carefully.

• Cleanup callback ordering
• Properly call WXT module's setup function when config changes
• Ensure port doesn't change between restarts
• Do a full restart when modules/* is saved during development, not just when wxt.config.ts is changed
• Make sure it's fixed: [Module API] Options not refreshed during development  #857
• Make sure it's fixed: [modules] Polluted manifest in successive build:manifest calls #863
• Make sure it's fixed: Modified wxt.config via wxt.hook is reset on dev reloaded #939
