vmauth: prevent from appending target_path_suffix multiple times

Summary by cubic

Fixes vmauth URL map generation to stop appending target_path_suffix more than once. The suffix is now applied exactly once per target URL, including when static.urls is provided.

• Bug Fixes
  • Combine URL prefixes instead of replacing them when static.urls is set, preventing duplicate suffixes.
  • Expanded tests for default and unauthorized users to cover named refs and static targets.
  • Updated CHANGELOG.
