P2 Badge Include trigger metadata in skill capabilities

When a repository-local SKILL.md relies on a trigger: field instead of description:, the new capability index drops the trigger because this extractor only stores name and description. The command/template now tell users that skill capabilities are derived from name, description, trigger, and path, so those skills get rendered with a generic scenario and agents lose the actual activation condition.
