Update from v0.1.1 to handle:

Some changes to Twitch API around removal of gifter name.
Twitch doesn't always send through currency details any more, they seem to be in USD more often now.
Patreon API found to not include any currency and NOT convert to USD either. Automatic handling of this is now questionable.

This integration is only lightly supported these days.
