This change refactors the http/stdio transports to extract MCP server creation into a reusable factory function. The MCP server is needed in other contexts beyond the transports, such as enabling deployment to Gram Functions. As part of this refactor we were able to remove many custom MCP types in favor of canonical alternatives from the official MCP SDK.
